This is the second Indy entry for team owner Fernandez. Super Aguri Fernandez Racing, co-owned by Fernandez and former Formula One driver Aguri Suzuki, filed an entry March 4 for rookie Kosuke Matsuura. That team also is a full-time participant in the IndyCar Series.
Fernandez will attempt to make his third Indianapolis 500 start but his first since 1995. His best finish of 21st came in 1995. Fernandez finished 28th as a rookie in 1994.
Mike Sales is the chief mechanic, with Gustavo Del Campo the team manager.
Fernandez, a native of Mexico City now living in Phoenix, will drive the No. 5 Quaker State Telmex Tecate Panoz G Force/Honda/Firestone. A backup car also has been entered.
The 88th Indianapolis 500 is scheduled for Sunday, May 30 at the Indianapolis Motor Speedway. Entries must be postmarked by midnight March 31. Qualifications for the 33-car starting field begin on Pole Day, Saturday, May 15.
"I am looking forward to the month of May and the Indy 500," said Fernandez, who competed full time in CART from 1994-2003. "We are hitting the ground running, and I know we will have a lot of support along the way."
Team co-owner Adrian Fernandez, a longtime open-wheel veteran, was named as the driver on the entry.
Fernandez Racing filed an entry for the 2004 Indianapolis 500 on March 12, one day after it announced it was moving immediately to the Indy Racing League(R) IndyCar(R) Series.
